I hereby request for an admission to the herein mentioned course offered by 'The Academy'. The information given above is true. I indemnify the institute against any errors in this form and also any future misgivings I may have about having proceeded with this admission. I also understand and accept that it is completely upto the discretion of the academy to accept or deny admission without having to provide explanation.
